As you see, the statement is printed in the blue box.
The responses, SA, A, D, SD, are printed on the right.

SA means you Strongly Agree with the statement.
A means you Agree with the statement.
D means you Disagree with the statement.
SD means you Strongly Disagree with the statement.

You should always check the best response.
Please go ahead now and check any of the responses on the sample question.

As you see, a box appears saying "click here to continue".

You may still change your response if you want to.
When you are ready to continue to the next statement,
click on the box with the arrow, like this: .
If you ever want to move back up to review
your answers, click the symbol that looks like this:

Some questions may ask follow-up questions.
Follow-up questions usually use the same format.
Some questions offer an empty text box for your comments.
To use the text box, just click inside the box, then start typing.

Completing the form --
